Grab and Go Lunches

  1. These sites are sponsored by Poudre School Nutrition Department and are distributed Monday – Friday from Noon until 1pm.  Due to the Stay at Home orders children are no longer required to be present to receive a lunch.  Parent/Guardian can request the number of lunches needed.   Breakfast for the next morning is also included in these lunches or as a separate bag depending on location.  Ask when you arrive.   Drive through only – no walk ups.

The Health and Wellness Center at Centennial High School& Virtual Care Services at Lincoln Middle School

What does our virtual care/telehealth model look like? 

  • We can diagnose and treat student-patients for all regular primary medical care (same types of visits/care that pediatricians and family medicine practitioners provide). 
  • We can also counsel student-patients and provide mental health services. 
  • We utilize HIPAA compliant audio/visual telehealth equipment to virtually connect student-patients (at their home) with a Health and Wellness Center Patient Care Provider

Dependiendo del lugar, el desayuno para la mañana siguiente también se incluye con estos almuerzos, o se entrega en una bolsita separada. Pregunten cuando lleguen. "Iniciativa especial": COVID-19 Mini-donaciones de Respuesta Rápida" - para aquellos que han perdido el trabajo o reducido las horas como resultado de COVID-19.

    What does our virtual care/telehealth model look like? 

  • We can diagnose and treat student-patients for all regular primary medical care (same types of visits/care that pediatricians and family medicine practitioners provide). 
  • We can also counsel student-patients and provide mental health services. 
  • We utilize HIPAA compliant audio/visual telehealth equipment to virtually connect student-patients (at their home) with a Health and Wellness Center Patient Care Provider

  • Distribución de almuerzos en bolsitas, patrocinada por el Departamento de Nutrición del PSD 
  • Debido a las órdenes de quedarse en casa, ya no se requiere que los niños estén presentes para recibir un almuerzo en bolsita. El padre de familia o el tutor legal puede solicitar la cantidad de almuerzos que necesite. Dependiendo del lugar, el desayuno para la mañana siguiente también se incluye con estos almuerzos, o se entrega en una bolsita separada. Pregunten cuando lleguen. Recojan el almuerzo en bolsitas SOLO desde su vehículo. No se tendrá acceso a pie.

    El Fondo de Emergencia para la Inmigrantes (FEI) de ISAAC– Este fondo es para las personas que no tienen derecho a asistencia gubernamental por carecer de estatus legal. "Iniciativa especial": COVID-19 Mini-donaciones de Respuesta Rápida" - para aquellos que han perdido el trabajo o reducido las horas como resultado de COVID-19.

  • Las solicitudes deben ser completadas por alguien de una de las organizaciones que figuran en la lista (que es una lista viva y la intención era cubrir todas las organizaciones representadas en este correo electrónico).     SERVICIOS

  • Podemos diagnosticar y tratar a los estudiantes-pacientes con la atención médica primaria regular (los mismos tipos de visitas/atención médica que los pediatras y los profesionales de medicina de familia brindan).
  • Servicios de terapia a los estudiantes-pacientes, y proporcionar servicios de salud mental.
  • Al ofrecer telesalud, utilizamos equipos audiovisuales que cumplen con la ley HIPAA de prácticas de confidencialidad, para conectar virtualmente a los estudiantes-pacientes (desde su hogar) con un asistente de atención al paciente del Health and Wellness Center (desde su hogar), y un profesional de la atención médica y/o de salud mental desde su hogar.
